Money Back Guarantee Package for In Vitro Fertilization (IVF)

In Vitro Fertilization Cycle Refund Or Pregnancy Plan (CROPP)

CROPP provides couples for whom IVF is medically necessary a chance to pursue their dreams of pregnancy with minimal financial risk. Qualifying patients may use their own eggs or donor eggs. All costs of the retrieval and transfer will be refunded if a pregnancy does not occur. Please review the individual packages for pricing information and terms of each plan. We also have capable, caring financial counselors to assist you in choosing the best package for you, and to answer any questions you may have. The doctor will decide if you meet the medical criteria for each pricing package.

FULL CYCLE REFUND IF PREGNANCY DOES NOT OCCUR

$13,500 3 cycles including all FETS egg provider < 34 years of age
$15,500 3 cycles including all FETS egg provider 35, 36, & 37 years of age
$17,500 3 cycles including all FETS egg provider 38 & 39 years of age

Frozen Embryo Transfers (FET) do not count as a cycle
Monitoring Visits (expect 6 - 10) - no charge to NCCRM patients

EXCLUDES:
Medical Screening & Initial Work-up: $600 - $1,000
Semen Analysis:  $100
Semen Freeze: $300
Intracytoplasmic Sperm Injection (ICSI): $2,000
Assisted Hatching: $1,000
Embryo Cryopreservation: $300 (Each cycle)
Annual Storage Fee (after first year): $200
Confirmation of Pregnancy Varies
Medications: approximately $3000 - $5000
SCSA - Sperm Chromatin Structure Assay test: $350

TERMS:

1. Required SCSA - Sperm Chromatin Structure Assay (SCSA®) test 
2. All three cycles must be completed within one year
3. All monies must be paid before medications are called to pharmacy
4. If pregnancy occurs contract is terminated 
5. Age of female is based on age at end of plan duration
6. The patient/couple must follow all medical protocols, schedules and directions prescribed by NCCRM doctors and nurses

MEDICAL CRITERIA
All patients must be aproved by physicians

1. Healthy uterine cavity
2. FSH & E2 within normal limits
3. Cannot be peri-menopausal
4. Males: sperm must be present in ejaculate at a level greater than 5 million total motile count
5. Donor sperm can be used
6. All medical and psychological evaluations (if applicable) must conclude the patient is healthy enough to undergo treatment and pregnancy
7. The patient/couple must follow all protocols, schedules and treatment plans
8. Some surgeries may exclude patient from plan
9. SCSA is required - results will determine if patient is eligible

ELIGIBILITY:

1. All medical criteria must be met
2. IVF cycles performed outside of NCCRM do not apply. But if you have undergone previous unsuccessful IVF cycles at NCCRM, the terms are as follows: If you have had two or more unsucessful cycles at NCCRM, you are not eligible to enroll in the refund plan. 
3. No Pre-implantation Genetic Diagnosis (PGD)
