Money Back Guarantee Package for In Vitro Fertilization (IVF) – In Vitro Fertilization Cycle Refund Or Pregnancy Plan (CROPP)

CROPP provides couples for whom IVF is medically necessary a chance to pursue their dreams of pregnancy with minimal financial risk. Qualifying patients may use their own eggs. If pregnancy occurs the contract is terminated with a viable pregnancy beyond 26 weeks. If a miscarriage occurs before viability during the FIRST or SECOND attempt, further attempts will be allowed AS LONG AS THE ONE YEAR TIME LIMIT HAS NOT EXPIRED. If a miscarriage occurs after 12 weeks on the THIRD attempt, NO REFUND OR ADDITIONAL CYCLES ARE OWED TO THE PATIENT. This plan is designed for couples who have no medical infertility benefits through their health insurance plan. We have capable, caring financial counselors who can answer your questions. Frozen Embryo Transfers (FET) do not count as a cycle

MONITORING MANAGEMENT:

$500 to cover daily cycle evaluation per cycle, non-refundable. NCCRM will bill insurance for monitoring and accept what insurance pays.

EXCLUDES:

  • Medical Screening & Initial Work-up: $1300 – $2000 (usually covered by health insurance)
  • Semen Analysis: $100
  • Semen Freeze: $200
  • Anesthesia: $250 – Mac Anesthesia with IV Sedation
  • Intracytoplasmic Sperm Injection (ICSI) if deemed necessary by physician and/or laboratory staff: $3000 for all cycles (non-refundable)
  • Assisted Hatching if deemed necessary by physician and/or laboratory staff: $1,500 total for all cycles (non-refundable)
  • Embryo Cryopreservation: $500 (Each cycle)
  • Annual Storage Fee (after first year): $325
  • Confirmation of Pregnancy Varies
  • Medications: approximately $2000 – $5000
  • SCSA – Sperm Chromatin Structure Assay test: $350
  • Blastocyst Culture: $250

TERMS:

  • Required SCSA – Sperm Chromatin Structure Assay (SCSA®) test
  • All three cycles must be completed within one year
  • All monies must be paid before medications are called into the pharmacy
  • Age of female is based on age at end of plan duration
  • The patient/couple must follow all medical protocols, schedules and directions prescribed by NCCRM doctors and nurses

MEDICAL CRITERIA

All patients must be approved by physicians

  • Healthy uterine cavity
  • AMH must be within normal limits
  • FSH & E2 within normal limits
  • Cannot be peri-menopausal
  • Males: sperm must be present in ejaculate
  • Donor sperm can be used
  • All medical and psychological evaluations (if applicable) must conclude the patient is healthy enough to undergo treatment and pregnancy
  • The patient/couple must follow all protocols, schedules and treatment plans
  • Some surgeries may exclude patient from plan

ELIGIBILITY:

  • All medical criteria must be met
  • IVF cycles performed outside of NCCRM do not apply. But if you have undergone previous unsuccessful IVF cycles at NCCRM, the terms are as follows: If you have had two or more unsucessful cycles at NCCRM, you are not eligible to enroll in the refund plan.
  • No Pre-implantation Genetic Diagnosis (PGD)
