IVF Shared Risk Program

Ideal for our medically eligible (self-pay) patients who are ready to grow their families, using their own eggs, and want the peace of mind that comes with shared financial risk.

Pricing

• Starts at a base price of $18,280 for the 50% Refund Program, and a base price of $23,815 for the 80% Refund Program. Price varies by age group.
• Base price does not include additional costs for monitoring, anesthesia, assisted hatching, cryo preservation, or management fee. These additional costs are paid individually at different stages of the cycle.

Money‑Back Terms

• Receive a 50% or 80% refund of the base price if no pregnancy occurs after all included cycles are completed within the program duration (18 months), as long as you meet all medical requirements of the program and adhere to all protocols.
• Some other terms and conditions may apply.

What’s Included

• Up to 3 IVF cycles, plus ICSI and all resulting FETs. Frozen Embryo Transfers (FET) do not count as a cycle- all embryos must be used before another retrieval cycle is done.
• Includes: facility fee, retrievals, blastocyst cultures, Frozen Embryo Transfers.

Additional Cost (not included in the package)

Medications, Medical Screening & Initial Work-up, Semen Analysis, Freeze & SCSA, IVF Education Class, PICSI or ZyMot, Annual Storage Fee for Embryos (after the first year), PGT-A(PGS) for Inherited Genetic Disorders or Chromosomes Screening with Family Balancing- optional, Confirmation of pregnancy after Transfer, Treatment associated with Hyper-Stimulation varies.

• Starts at a base price of $30,300 for the 50% Refund Program, and a base price of $39,440 for the 80% Refund Program. 
• Base price does not include additional costs for donor medications, monitoring and compensation, assisted hatching, cryo preservation for 2nd and 3rd cycles, or management fee for all three cycles. These additional costs are paid individually at different stages of the cycle.

Program Eligibility

• All medical criteria listed below must be met. 
• IVF cycles performed outside of NCCRM do not apply. 
• If you have undergone previous unsuccessful IVF cycles at NCCRM, please review the terms below.
• If you have had two (2) or more unsuccessful cycles at NCCRM, you are not eligible to enroll in the shared risk program.
• You may enroll in The Shared Risk Program one time only.

Medical Criteria

• The Medical Director MUST approve all Patients.
• Healthy uterine cavity.
• Anti-Mullerian Hormone (AMH) 2.5 or more.
• Prolactin & TSH within normal limits.
• Cannot be pre-menopausal.
• Male sperm must be present in the ejaculate at a level greater than 5 million total motile count.
• Donor sperm can be used.
• All medical and psychological evaluations (if applicable) must conclude the patient is healthy enough to undergo treatment & pregnancy.
• The patient/couple must follow all medical protocols, schedules, and directions given by an NCCRM doctor or nurse.
• Some surgeries may exclude the patient from the plan.
• SCSA must be done, and the results of the SCSA will determine if the patient is eligible.

Program Terms

• All three cycles and all FETS must be completed within 18 months.
• Financial responsibility for a certain cycle must be paid before medications are called to the pharmacy.
• SCSA required.
• Intracytoplasmic Sperm Injection (ICSI) & Assisted Hatching will be done.
• The contract is terminated when 26 weeks of pregnancy are completed on the 1st or 2nd cycle, and 12 weeks on the 3rd cycle.
• Patient’s age by which she is meeting the plan criteria is based on her age at the end of the plan duration. The patient/couple must follow all medical protocols, schedules, and directions prescribed by or ordered by NCCRM doctors or nurses.
• All consents and/or legal agreements must be signed by all parties involved.
• Any services provided outside of NCCRM are NOT eligible for refund, nor is NCCRM responsible for any services provided outside of our office.
• Packages apply only to self-paying patients.
• No exception to the Shared Risk Program terms.
